Forms

A form is a collection of questions (called blocks) that you publish and share with respondents. Each form belongs to a workspace and has its own settings, theme, integrations, and analytics.

Blocks

Blocks are the building units of a form. Zevform supports 24+ block types including text inputs, choice fields, ratings, file uploads, signatures, payments, and more. Special blocks like Welcome and Thank You control the start and end screens.

Workspaces

Workspaces organize your forms and team. Every account starts with a default workspace. On paid plans, you can create multiple workspaces and invite team members with role-based access (Owner, Admin, Member).

Themes

Each form can have a custom theme controlling colors, typography, button styles, and background. Choose from presets or build a fully custom look with gradient and image backgrounds.

Logic

Conditional logic lets your form adapt based on answers. Set rules like “If rating < 3, show follow-up question” or “If answer is Yes, skip to section 4”. Logic supports AND/OR conditions and jump targets.

Answer Piping

Reference previous answers in later questions using {{questionId}} syntax. For example: “Thanks , how was your experience?”

Integrations

Connect forms to external services. Zevform supports Google Sheets, Notion, Slack, Discord, webhooks, and email notifications. Integrations are configured per form.
