Question Types

Zevform supports a wide range of question types to collect any kind of data.

Text Inputs

Single-line text input for names, titles, and brief answers.
Multi-line paragraph input for detailed feedback, comments, and descriptions.
Email-validated input that ensures respondents enter a valid email address.
Phone number input for collecting contact numbers.
Website URL input with format validation.
Numeric input with optional min/max constraints.
Calendar date picker for scheduling and date collection.

Choice Fields

Single-select option list. Respondents pick one answer from a set of choices.
Multi-select options. Respondents can select multiple answers.
Simple binary choice with yes and no buttons.

Rating & Scale

Star rating scale (typically 1-5) for satisfaction and quality measurements.
Numeric linear scale (e.g. 1-10) with customizable range.
Net Promoter Score (0-10) with customizable left/right labels like “Not likely” and “Very likely”.
Range slider with min, max, step values, and optional labels.

Advanced

Grid question with customizable rows and columns for multi-dimensional data collection.
Drag-to-reorder list where respondents rank items by preference.
File picker with configurable max files, max size, and accepted file types.
Drawn signature capture for consent, agreements, and approvals.
Structured address collection with optional autocomplete.
Payment block with checkout link integration. Supports Stripe, Dodo Payments, and Polar. Configure product name, description, amount, currency, and button text.
Computed field using formulas and answer references. Use arithmetic expressions to calculate totals, scores, and derived values.

Intro screen shown before the first question. Supports custom title, description, button text, and cover image.
Completion screen shown after the last question. Supports custom messaging and images.

Common Settings

Every question type supports:
  • Title — The question text shown to respondents.
  • Description — Optional helper text below the title.
  • Required — Whether the question must be answered to proceed.
  • Logic — Conditional visibility and jump rules.
