Creating Forms

Zevform offers three ways to create a form.

AI Generation

The fastest way to create a form. Describe what you need in natural language and Zevform’s AI generates a complete form with appropriate question types, options, and structure.
  1. Click New form in your dashboard.
  2. Select AI Generate.
  3. Enter a prompt describing your form, e.g. “Employee satisfaction survey with work-life balance, management, and growth questions”.
  4. Optionally adjust the number of questions.
  5. Click Generate.
AI generation uses credits from your plan. Free plans include 10 credits/month, Pro includes 100.

From a Template

Choose from 50+ professionally designed templates across categories like Feedback, Survey, Lead Generation, Registration, Orders, Quiz, and HR.
  1. Go to Templates in the sidebar.
  2. Filter by category or browse all.
  3. Click a template to preview it.
  4. Click Use Template to create a copy in your workspace.

Blank Form

Start from scratch with a blank canvas.
  1. Click New formBlank form.
  2. Use the + button to add blocks.
  3. Choose from 24+ question types.

Form Lifecycle

Forms go through these states:
  • Draft — Form is being edited and not yet accessible to respondents.
  • Published — Form is live and accepting responses.
  • Closed — Form is no longer accepting responses (manually closed, date expired, or submission limit reached).
You can also duplicate any form to create a copy, or delete forms you no longer need.
